- Register for middle school sports and activities through Final Forms, a secure, user-friendly platform.
- All students in middle school can participate in sports through the district’s no-cut policy.
- Students may participate in one sport/activity per season
- Students cannot practice or participate in competitions until all required forms have been completed and submitted through Final Forms. Students also must have a valid physical to start practice. Please download our Physical Form to take to your Doctor or an Urgent Care Clinic. We do not accept Physicals from CCYF, Boy Scouts or a physical not on our form.
- If you are on free or reduced lunch, please call the Business Office for more information.
- All fines and fees must be paid before a student can participate in an Activity
- EARLY RELEASE DAYS: Athletes will go to a supervised Study Skills until Practice starts at 3:45.
- SNACKS: Most Athletes bring some sort of a snack to eat after school before practice or before a game.
FALL SPORTS 2023-2024: Registration on Final Forms begins August 1st, 2023.
Practice begins August 23rd. However, that could change and times are pending. An email will be sent
out a few days before that with more detailed information.
Please pay your Sports Fees and ASB Card at time of Registration. There is a form you must electronically sign off on called
ONLINE PAYMENT. It will give you the link to Skyward to make your payments. Can pay for Yearbook at the same time.
Also, please have your student sign their forms as well so they can be completely cleared before practice starts.

- All athletic fees and fines must be paid in order to compete. Financial assistance is available.
- Each sport requires a valid ASB card ($10 each calendar year) and sport-specific fee of $20. These funds are used to help defray the cost of equipment, cleaning/repair/replacement of uniforms, transportation, officials and other expenses.
